Partnership HealthPlan of California is a nonprofit community-based health care organization that contracts with the state to administer Medi-Cal benefits through local care providers to ensure Medi-Cal recipients have access to high-quality comprehensive cost-effective health care. Partnership provides quality health care to approximately 900,000 individuals. Beginning in Solano County in 1994, Partnership now provides services to 24 Northern California counties.

Adventist Health Clear Lake

Adventist Health Clear Lake is a 25-bed critical access medical center in Clearlake, California, at the intersection of hope and healing in Lake County. Adventist Health Clear Lake is situated in one of the most beautiful areas of Northern California and proudly offers primary and specialty care services at locations in neighborhoods from Lakeport to Middletown.

With fourteen medical office locations, more than 90 providers and 700-plus associates, we strive to ensure quality healthcare is accessible to the approximately 65,000 citizens in our county. We meet the healthcare needs of this population by offering incredible quality, highly-trained specialists, and bright and welcoming facilities that house the latest technology.

Lake County Tribal Health Consortium, Inc.

Lake County Tribal Health Consortium is an Indian Health Service facility, part of the Federal Health Program, founded in 1961 and owned by six local tribes. Located in a rural area in Lake County, the consortium serves a patient population of 14,000 with a dedicated staff of 338. The facility operates across three clinic sites: Lakeport, Southshore, and a Pediatric Clinic, utilizing the NextGen electronic medical record system. Comprehensive services provided include medical, dental, pediatrics, obstetrics, comprehensive healing and pain management, human services, public health and outreach, pharmacy, purchased referred care, transportation, a diabetes specialty care clinic, and physical therapy.

Mendocino Coast Clinics

Mendocino Coast Clinics is a Federally Qualified Health Center, located in Fort Bragg on the beautiful Mendocino coast. We have been serving our community for nearly 30 years, providing a wide range of services to patients of all ages, regardless of insurance or residence status. MCC offers medical, dental, behavioral health, sexual and reproductive health, and pediatrics services, as well as a full service opioid use disorder treatment program. We also offer chiropractic and acupuncture services. We are proud to serve our community with the best care anywhere!

North Coast Opportunities, Inc., known as NCO, is a Community Action Agency that has been serving Lake and Mendocino Counties for over 50 years. Community Action is a program founded by the federal government in 1964 to fight poverty by empowering the poor to become self-sufficient. In support of this War on Poverty, NCO serves community members through over twenty-five programs that range from childcare to workforce development, food access to emergency preparedness and more.
